Brain tumor imaging is a test bed for various image biomarkers (IBs), including the current new IB of artificial intelligence (AI). However, while some IBs are routinely used for testing and clinical decision-making for patients, most IBs are not. Why? New IBs must surpass the translational cap to be used as a clinical decision-making tool, and this follows a clinical roadmap. By presenting various clinical scenarios of IBs that have adopted, this will demonstrate a clinical roadmap of IBs in brain tumors and how they can become the next decision-making tool, including both IBs and AI.
