How Do I Interpret Liver Lesions in Cirrhosis?

Synopsis

The differential diagnosis of hepatic observations is often challenging. Liver MRI has been increasingly used for characterization of hepatic observations.

Target audience

The most common primary malignancy is hepatocellular carcinoma (HCC). In our practice, it would be our primary goal to decide whether an observation is HCC or cirrhotic nodule in cirrhosis. However, there is an overlap of imaging features between cirrhotic nodules and HCC. Also, malignancies other than HCC can develop in cirrhosis. In addition, HCC sometimes mimics other malignancies by showing atypical appearances. In this talk, we will review cases of hepatic observations commonly found in cirrhotic liver, focusing on features in gadoxetic acid-enhanced liver MRI. Both typical and challenging cases will be discussed using terms of LI-RADS v.2018 in different clinical settings. At the end of this talk, participants will be able to understand different pathologies in the cirrhotic liver and describe characteristic features.
